Student Information Sheet Freebie

At the beginning of the school year, I always make sure that I have all the parents fill out a student information sheet.  I have two different ones that I made.  The first one is more detailed, so that I may get to know the student better.  However there are times that some of the information needs to get updated.  So, I made another one for parents to use when there is a change of a phone number, address, etc.  I hope you find this student information sheet useful!

Father's Day 'Super Dad' Party

Happy Father's Day weekend!  Wow...we did it, we were able to return back from our track break in the middle of the week and still manage to get everything ready for our Father's Day Celebration!!  YEAH!!  Well, I kept thinking of a theme for the celebration and then it clicked on me.  'Hey, my kiddos really love minions...so why not have a super dad party!'  The joy and cheers they kids did when they found out what the theme was, lol.  Here are a a few pictures of our Super Dad Party!
Here is one of the placemats for our Father's Day Celebration.  
I wanted the outside of our hallway to be the same as our Mother's Day Tea Party so that the Dad's had something to read while they waited outside our door.  
When I opened the door, each student gave their Dad a big hug and escorted them to their seat.  The students sang 2 songs and we watched our Father's Day slide show.
My students shared their Father's Day gifts while they waited for their turn to get some refreshments and goodies to eat.  We decorated a large manila envelop with a drawing of a minion and wrote 'Happy Father's Day!  You are 1 in a Minion!'  Inside the envelope we put a card that they made, Daddy booklet, Father's Day coupons, Trophy craft that said 'World's Best Dad,' and a picture frame with their handprints.
It was a day filled with fun and excitement...however I am super tired.  Good thing we won't have another celebration till our kinder graduation, cause parties do take a lot of preparation and work, heehee!:)
